Reports Landing Page

The Reports Landing Page screen lists all the reports that are available within the EIM system. The user can click on any of the hyperlinks to navigate to the corresponding report request screen to request a report. The user is also able to navigate to their Reports Inbox to access offline reports that the user requested.

Field Names	Field Description
Reports Inbox	Click the link to access the user's Reports Inbox, where offline reports requested by the user are stored.
Incident and Complaint Details by Individual	Click the link to request a report that allows users to view incident and complaint details grouped by individual.
Incident and Complaint Details by Service Location	Click the link to request a report that allows users to view incident and complaint details grouped by provider service location.
Incident and Complaint Custom Report	Click the link to configure and request a custom report based on specific fields and responses.
Incident History by Individual	Click the link to request a report containing incident information for a single participant by MCI number. Note: This report is visible to OLTL users only.
Incident Target Report	Click the link to request a report containing information on the target of one or more incidents. Note: This report is visible to ODP-ID/A and ODP-BSASP users only.
Incident Management Review Report	Click the link to request a management level report based on specific fields and responses.
Incident Corrective Action Report	Click the link to request a report containing corrective actions based on specific fields and responses. Note: This report is visible to ODP-ID/A, ODP-BSASP, and OLTL users only.
Medication Error Report	Click the link to request a report containing information about medication error incidents. Note: This report is visible to ODP-ID/A and ODP-BSASP users only.
Incident Mortality Review Report	Click the link to request a report containing information on the review of incidents with a primary category of ‘Death’. Note: This report is visible to ODP-ID/A State and Region users and OLTL State users only.
Incident Reporting Overview Dashboard	Click the link to view metrics related to compliance of first-pass submissions of Incident Report documents. Note: This dashboard is visible to ODP-ID/A and ODP-BSASP users only.
Restraint Dashboard	Click the link to view metrics related to restraints with a focus on restraints which were conducted improperly. Note: This dashboard is visible to ODP-ID/A and ODP-BSASP users only.
Medication Error Dashboard	Click the link to view metrics related to medication error incidents. Note: This dashboard is visible to ODP-ID/A and ODP-BSASP users only.
