Incident Classification
The Incident Classification allows users to classify an incident and capture the incident's date and time information. This screen also allows certain users to assign an investigator to an incident. Note: Certified Investigators are not used by OLTL; Service Coordinators and Managed Care Organization staff conduct internal investigations to obtain information for the final section of the incident report.
Criteria for Reporting an Incident to Adult Protective Services (APS): Incidents must be reported to APS when:
- The individual is 18-59 years old (based on the date of birth field), and
- The incident primary category is abuse, neglect or exploitation.
When both of these conditions are met, the user should report the incident to APS, and select 'Yes' to 'Was this incident referred to Adult Protective Services (18-59 years of age)?'.
Criteria for Reporting an Incident to Older Adult Protective Services (OAPS): Incidents must be reported to OAPS when:
The individual is over 60 years old (based on the date of birth field), and
- The incident primary category is abuse, neglect or exploitation.
When both of these conditions are met, the user should report the incident to OAPS, and select 'Yes' to 'Was this incident referred to Older Adult Protective Services (60+ years of age)?'.

Select the category of the incident from the drop-down list (i.e. death, abuse, neglect, etc.) |

Select the date when the primary category of the incident occurred. |
Secondary Category |
Select the secondary category of the incident to provide additional information such as accidental, neglect, physical abuse, etc. |

Select the date when the secondary category occurred. |

Optionally, click this button to determine if an investigation by a certified investigator is required for the incident. Note: Because OLTL does not use certified investigators, the use of this button is not required. |
